Abstract

This simulation-based scenario was designed to identify and manage anaphylactic shock in the pre-anesthesia care unit. Anaphylaxis is a rare but potentially life-threatening allergic reaction. During the perioperative period, an anaphylactic reaction can cause hypotension, and the most common precipitating agent is antibiotics. Rapid identification and management of anaphylaxis play a major role in patient prognosis. In addition to hypotension, patients often present with rash/hives, facial edema, bronchospasm, stridor, and wheezing. Patients should be treated immediately with the removal of the offending agent if possible, vasopressors (especially epinephrine), supplemental oxygen, and volume resuscitation. This scenario should be covered in medical resident training, especially in the department of anesthesiology.
